Instructions for creating and account and registering for your upcoming appointment using our Patient Portal! 

First print this page to guide you through the process.

Now, click this link to take you to the Patient Portal.

The first thing you need to do is “Create an account”. You will choose a “User ID” and a “Password”.  Be sure to save this information and keep it secure.  You will use the Portal for many things, including viewing you personal exam information.

Once you create your account – go ahead and LOG IN using your newly created “User ID” and “Password”.

VERY IMPORTANT!  At this time you must COMPLETE YOUR REGISTRATION.  This will take 15-20 minutes.  You will enter your demographic information, insurance information and medical history as well as your current symptoms.  YOU WILL ONLY HAVE TO DO THIS ONCE. IN THE FUTURE – ALL YOU WILL NEED TO DO IS EDIT THE INFORMATION IF THERE ARE CHANGES.

By doing this, you will save precious time the day of your appointment.  If you choose not to COMPLETE YOUR REGISTRATION you must arrive 20 minutes early for your appointment.

The Portal is also a great way to send us secure messages, pay your bill online or schedule your next appointment.

Every time you are scheduled for an appointment, log in to view and edit information as needed and give us your current symptoms or reason for visit.
